Coronavirus liability release
Why this is necessary
Due to the outbreak of coronavirus COVID-19, we are taking extra precautionary measures while carrying out treatments, which is both for staff and our clients’ safety. Our measures include; increased sanitisation in between client sessions and the wearing of Personal Protection Equipment (PPE) that has been recommended by the government, while a session is in progress.
We recommended that you don’t bring any personal belongings into your session with you and that you come alone.
We are asking all clients to read and confirm that the following declaration is true before they attend any sessions.

I confirm and agree to the following.
I can confirm that not I, nor any of my household members, have displayed or experienced any of the following symptoms within the last seven days.

A loss, or change, of taste or smell

A continuous cough

Had difficulty breathing

A fever
I can confirm that not I, nor any of my household members, have been diagnosed with COVID-19 within the last 30 days.
I can confirm that not I, nor any of my household members, have knowingly been exposed to anyone diagnosed with COVID-19 within the last 30 days.
I can confirm that not I, nor any of my household members, have travelled outside of the country or to a city, that has been considered a hotspot for COVID-19 within the last 30 days.
I understand that this business and my therapist cannot be held liable for any exposure to the virus or any other contagion caused by misinformation on this form for the health history provided by each client.
I confirm that by ticking the checkbox or by attending my booking that I:

Our declaration
Please know that the therapist and everyone else within this facility agreed that they would abide by the same standards asked of you. We also affirm that we have improved and expanded our sanitisation protocols to a higher, more rigorous standard.
We will do all we can to help fight the spread of COVID-19 and other communicable conditions.
